- Litigation case –
A failure and plant shutdown of a very large slug catcher overseas which caused a multiyear shutdown. Ken’s involvement was to rebut the plaintiff’s metallurgical expert’s report, and comment on where that report erred. This involved reviewing massive amounts of fabrication reports, operational reports, inspection reports, and failure analyses. - Litigation case –
The failure of down hole tubulars in an offshore well which resulted in abandonment of the well. Traced all material test records, inspection records, and shipping records to establish which vendor provided the defective casing. Ken testified regarding his findings at the trial. - Litigation case –
A case involving the failure of a propane line resulting in an explosion and fatality. Ken determined that the cause of the external corrosion was due to an improperly installed isolation flange where the propane line entered the plant. This improper isolation resulted in the cathodic protection anodes being consumed very rapidly allowing external corrosion to perforate the propane line. Ken testified regarding his findings at the trial.
